Social prescribing
Social prescribing is a service that works alongside the GP surgery. The aim is to connect you to community and voluntary services and /or activities to help improve your health and wellbeing. Within Carlisle Healthcare we have two Social Prescriber's Rachel who works full time and Stacey who works part time.
The types of support that they provide are:
- A space to explore issues that are negatively affecting your physical and mental health and overall wellbeing.
- Practical, helpful and non-judgemental support to help you identify and acheive goals.
- A confidential space to talk and think about what matters to you.
- A place to receive advice, guidance and advocacy.
- A service that can give you information about/refer you to local groups, community activities and service
- A service to help people feel connected and take control of their own wellbeing.
